Purpose: (General description and summary)

Clifford Apartments QMHA Mental Health Associate provides clinic and community-based interventions designed to stabilize high-need individuals at the Clifford Apartments in Multnomah County. The Mental Health Associate works as part of a clinical team to implement, monitor, and document services to individuals enrolled in the program. Mental Health Associate follows procedures, outcome goals, and reporting requirements as outlined by Luke-Dorf's contracts with property management and owners, HUD, Multnomah County Department of Community Justice and Joint Office of Homeless Services. Mental Health Associate will be expected to work independently both in the clinic and community, as well as transport individuals to and from appointments and advocate for consumers.

Case Management

QMHA Mental Health Associates collaborate closely with QMHP Mental Health Clinicians to provide services as outlined on the treatment plan for individuals who meet the criteria for rehabilitation programming and other programs. The team members provide comprehensive wrap around services to assist individuals to meet their basic needs, accomplish goals as outlined in the treatment plan, attain the highest level of independence possible, and continuously strive for mental health stability.
